About this role
The RN Case Manager facilitates care along a continuum through effective resource coordination. This role helps patients achieve optimal health, access to care, and appropriate utilization of resources, balanced with the patient’s resources and right to self-determination. Ensure care is provided at the appropriate level based on medical necessity.
Assess patients for transition needs to promote timely throughput, safe discharge, and prevent avoidable readmissions. Conduct accurate medical necessity screening and submit for Physician Advisor review. Perform care coordination and transition planning assessment and reassessment.

Requirements
- Graduate of an accredited school of nursing
- 2 years of acute hospital patient care experience
- Active RN license for state(s) covered
- Academic degree in nursing (bachelor's or master's) preferred
- Acute hospital case management experience preferred
- Accredited Case Manager (ACM) preferred
Responsibilities

- Implement or oversee implementation of the transition plan
- Lead and facilitate multi-disciplinary patient care conferences and manage concurrent disputes
- Identify and refer complex patients to Social Work Services
