Provide nursing health assessments to identify and resolve client health and safety concerns. Monitor medication effectiveness, side effects, and treatment plans using a person-centered approach. Make recommendations to ensure optimal care.

Assist in developing community supports for individuals with complex medical needs. Monitor medical charts, nursing guidelines, and health care plans per the Person-Centered Plan. Administer medications and attend consultations on health concerns.

Requirements

  • Valid Registered Nurse License, state of Michigan
  • Associate's degree or better in Nursing
  • Valid Michigan Driver's License
  • Experience working with the I/DD Population
  • Familiarity with person-centered planning and medication monitoring
  • Ability to link with health care professionals and interpret physician orders

Responsibilities

  • Provide nursing health assessments to identify and resolve client’s health and safety concerns
  • Monitor effectiveness and side effects of medication, standards of care/treatment plans according to person-centered approach, and make recommendations
  • Assist in developing and linking to community supports for addressing health care needs of individuals with complex medical needs
  • Provide periodic review concerning health care treatment
  • Monitor and evaluate medical charts, nursing care guidelines, health care plans, treatment and response as outlined in the Person-Centered Plan
  • Develop the scope, frequency, and duration of nursing services based upon the individual’s Person-Centered Plan
  • Attend special consultations regarding individuals and their health care concerns per the Person-Centered Plan
  • Administer medication per the Person-Centered Plan and interpret physician’s orders
