About this role

The Clinical Registered Dietitian I assesses the nutritional status and develops evidence-based nutrition care plans for patients across the lifespan. Conditions include Type 2 diabetes, hyperlipidemia, hypertension, metabolic syndrome, and weight management. This role supports Adelante’s team-based primary care medical home model of coordinated, patient-centered care.

Daily responsibilities involve using the Nutrition Care Process for comprehensive nutrition assessments and documenting care plans in the electronic health record. The dietitian establishes SMART goals with patients and schedules follow-up appointments as needed. They evaluate and educate on nutrition-related medications, dietary supplements, and drug-nutrient interactions.

Collaboration occurs with the integrated care team, including behavioral health consultants, registered nurses, care coordinators, medical assistants, and primary care providers. Clear communication ensures proper patient care plans. The position emphasizes quality assurance and referrals to interprofessional resources.

Requirements

  • Bachelor’s degree in Dietetics or nutrition-related degree required; Master’s degree preferred
  • New grad to two years’ experience in community or clinical nutrition education programs or completion of a dietetic internship with RD/RDN credentials
  • Current credentials as a Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN)
  • Certification to perform CPR and AED through American Heart Association or Red Cross guidelines
  • Valid Level One Fingerprint Clearance Card issued by Arizona Department of Public Safety
  • Experience with motivational interviewing
  • Bilingual (English/Spanish) strongly preferred
  • Valid Arizona driver’s license and current auto insurance

Responsibilities

  • Assess patient’s nutritional needs using the Nutrition Care Process and document nutrition care plan in the electronic health record
  • Develop appropriate nutrition therapy interventions
  • Communicate and coordinate relevant patient information amongst the care team
  • Evaluate and educate patients on nutrition-related medications, dietary supplements, and drug-nutrient interactions
  • Develop, write, and review written nutrition-related patient education materials
  • Educate interdisciplinary staff on dietetic nutrition education guidelines
  • Refer to appropriate interprofessional team members, resources, or programs
