The Reactivation Care Centre (RCC) is a specialized short-stay program supporting patients who no longer require acute care but need rehabilitation before returning home. The interdisciplinary team delivers goal-directed assessment, treatment, and functional reactivation to prevent deconditioning and ensure safe transitions. Grounded in compassion, the RCC promotes mobility, engagement, and recovery in a purpose-designed environment.
The Occupational Therapist plans, implements, and evaluates client-centered occupational therapy services consistent with St. Joseph's mission and values. They assess physical, emotional, social, and cognitive capacity, occupational demands, and environmental factors. Interventions enable participation in everyday activities to promote health and well-being.
Key functions encompass clinical, professional, and non-clinical activities shaped by program needs and client population. Tasks align with College of Occupational Therapists of Ontario competencies, including professional responsibility, critical thinking, and effective communication. The role involves collaboration within an inter-professional team focused on seniors with complex conditions.
Affiliated with McMaster University and Mohawk College, the Occupational Therapist engages in teaching and research activities. They demonstrate adaptability, analytical thinking, and commitment to professional development. This position supports optimal patient outcomes and person-centred care in acute physical medicine settings.
Requirements
- Degree in Occupational Therapy from a recognized university and registration with the College of Occupational Therapists of Ontario
- Minimum one year recent and related experience in variety of physical medicine settings
- Demonstrated interest and experience working with clients with multiple complex medical conditions, particularly older adults
- Demonstrated knowledge, sensitivity and expertise in the care of seniors and their families
- Current BCLS/HCP certification
- Strong team and communication skills
- Proven ability to work as an effective member of an inter-professional team
- Excellent verbal and written communication skills
Responsibilities
- Plan, implement, and evaluate client-centered occupational therapy services
- Assess and diagnose physical, emotional, social, and cognitive capacity and performance
- Determine occupational demands and environmental factors for interventions
- Enable participation in activities of everyday life to promote health and well-being
- Assess inpatients to determine strengths and weaknesses cognitively, perceptually, physically, and psycho-emotionally
- Engage in clinical, professional, and non-clinical activities per program needs
- Collaborate effectively within interdisciplinary team to support patient transitions
- Participate in teaching and research activities due to university affiliations
